Do you want to upgrade your membership type or insurance?

Do you want to add insurance, WFOT membership or the Member Assistance Program (MAP) to your current CAOT membership?

If you answered yes to either of these questions, please download the Membership Upgrade Form and send it in to the CAOT national office.  Please refer to page 2 of the form for the costs associated with these upgrades.
