Occupational therapy documentation conveys the results of assessment, details of intervention and client outcomes, and our professional opinion and plan to a wide variety of stakeholders. This webinar will present tips for writing clear, concise, and effective chart notes using the SOAP note format (Subjective, Objective, Assessment, Plan). Although the SOAP format is not used in all settings, the clinical reasoning process that underlies this method of note writing can be adapted for use in most occupational therapy practice settings across the health care continuum. This webinar will be useful for recent graduates and experienced occupational therapists who wish to improve their documentation skills.
