Goal setting and action planning are evidence-based tools that are becoming elements of practice across many settings - from primary care to rehabilitation and community care. This webinar will briefly introduce the theoretical basis underpinning their use, focus on practical tools for use by practitioners and report the current evidence of effectiveness.
